Gait Analysis

Gait analysis is a sophisticated laboratory technique by which modern electronics are used to incorporate information from a number of inputs to illustrate and analyze the dynamics of gait. It describes for the clinician (physician, surgeon, therapist) in quantitative and dynamic terms the movement of the body and its limbs and the changing relationships of one extremity to other extremities during motion (e.g. walking, leg raising). It helps the clinician judge what are the forces resulting in a dysfunction in movement of a limb and what is the result of compensating for the dysfunction; to put it another way, what is cause and what is effect.

Gait analysis has become particularly useful to the surgical team when decisions need to be made about the applicability of a surgical procedure for correction of a faulty gait. It is also valuable after surgery to learn whether the dysfunction has been corrected and how motion of the treated limb is now effecting the dynamics of walking.

Kinemetrix Motion Analysis System

 Kinemetrix Motion Analysis System is a video-based 3D motion analysis system which accurately quantifies function and provides objective data the professional may use to justify or evaluate treatment.

Accurate assessment of gait in orthopaedics has been one of the most difficult objective measurements to make in the clinic. The system allows the clinician to monitor the progress of disease or treatment of patients. It is the ideal instrument for those working in orthotics, prosthetics, orthopaedics, rheumatology, neurology and rehabilitation.

 Areas of successful application for motion analysis system include human performance, injury and rehabilitation assessment, as well as applied research in the areas of orthopedic medicine, therapeutics, and physical training.

Once the test has been performed the data may be manipulated and viewed in a variety of different ways. The motion of each individual marker may be investigated, and a movable cursor can be used to identify its instantaneous position, velocity, and acceleration at every point in time throughout the test.
For each parameter, the maximum and minimum values and the integrated area may be calculated over user-defined sections of the data. The behavior of two or more markers may also be studied simultaneously, allowing the calculation of joint angle, and angular velocities and accelerations. By defining the relationship between the markers on the test subject, stick figures may be displayed on the screen moving in real time. When three dimensional information is collected using two or more cameras these figures may be rotated about their axes, enabling the operator to view the motion from any angle.

 Bertec's Force Plate

 

 

  One of the most common forces acting on the body is the ground reaction force, which acts on the foot during standing, walking or running. This force vector is three dimensional, consisting of two shear components (X and Y) acting parallel to the ground plus a vertical supporting force (Z). These forces can be measured using a multi-component force transducer. This transducer is commonly known as a force platform. The force platform also provides the location of the center of pressure of this ground reaction vector using the relative vertical forces seen at each of the transducers.

  It is often valuable to combine the ground reaction force with the video data. These vectors can be studied with the goal of improving sports performance, evaluating injuries or the effects of disease. The vectors module provides the clinician with the tool to observe any deviations between normal leg and foot alignments with the resultant force vector.

Davinsa Treadmill (DST 2000) and Cardiac Exercise

 

 Motion analysis of running and its measurements are restricted by the limited length of walkway. To overcome this problem, we use a treadmill in front of the cameras so that all markers can be captured. Treadmill based gait analysis offers a potential advantage over the traditional methods, as well as many other benefits; such as controlling environmental constraints, adjusting the desired speed, acquiring more consecutive strides, and improving trial-to-trial and session reliability.

 

 At the Biomechanics Lab, we have a DST 2000 Treadmill (made in Iran) with the following specifications:

 

·         Effective running surface: 48 . 21 Cm

·         Speed Range: 0 to 20 KMPH

·         Incline Range: 0 to 20%

·         Emergency stop key

·         Electrocardiograph model Comp10 with capability of recording 12 channel ECG signals.

·         Real time heart rate and target heart rate monitoring.

 

·         سیستم تردمیل Davinsa : این سیستم ایرانی با قابلیت تنظیم شیب ، سرعت و زمان مورد نیاز برای انجام آزمون ، امکان انجام بررسی های بیو مکانیکی همراه با سیستم آنالیز حرکت را فراهم می آورد . سیگنال قلبی نیز در این سیستم قابل ثبت می باشد.

 

 

 

Biometrics Goniometers

Biometrics goniometers are designed for the measurements of limb angular movements. They are ideal for quick, simple, and accurate measurement of joint movement in multiple planes. The sensors can be comfortably worn under clothing, without hindering the actual movement of the joint.

 

The goniometers have two separate output connectors, one is measuring movements in sagittal plane (flexion/extension) and the other movements in vertical plane (Abd/Add). When used to measure a single axis joint such as the knee or elbow, or when measuring a single plane of a twin axis joint, simply connect one channel, the other remains redundant.

Biodex Isokinetic System 3 Pro

 Isokinetic equipments are instruments that allow to perform muscular exercises at a constant speed along the entire range of motion. After an acceleration phase, once reached a given angular velocity, therefore, since the maximum force varies according to the articular angle, also the mechanical resistance from the muscle will vary with the variation of the articular angle, thus the muscle meet the resistance that remains constant along the whole range of motion.

In rehabilitation, it can be used in all the re-educational treatments where a muscular functionality recovery is needed. 

 The Isokinetic Exercise is in fact finely adaptable and can therefore be used with extremely weak or very strong patients.

 

 

 Biodex Isokinetic System 3 pro is the isokinetic multi joint rehabilitation system. System 3 pro features five different operating modes including passive, isokinetic, isometric, isotonic and eccentric. Attachments to test all lower extremity and upper extremity joints are available in system 3 pro including ankle, knee, hip, wrist, elbow and shoulder. Separate attachments for testing the trunk and for closed kinetic chain testing are available in system 3 pro. MT8 Radio Telemetry System

 

Electromyography (EMG) is the study of muscle activity by measuring electrical signal associated with the contraction of a muscle. Clinicians and researchers can apply the study of EMG to the fields of neurology, surgical assessments, functional capacity evaluations, ergonomics and overuse injuries and chronic pain evaluations.

 

In MT8 Radio Telemetry System (or kinesiological EMG), data analysis using Myo-Dat software can be used simultaneously with other compatible software.

Myo-Dat is a sophisticated data collection package that automatically detects the gain settings on the MT8 receiver and produces fully calibrated analysis of EMG and other transducers. Analysis includes frequency spectra, power spectra, center frequencies, enveloped EMG, quantified EMG, integrated EMG, correlation curves between two signals and EMG protocols. Real time biofeedback of up to eight channels is also included.

The shortcoming of MT8 system is its limited sampling rate of 625Hz while acquiring 8 channels simultaneously. This problem has been solved by a custom-made DAQ system at the Lab.

Micromed Brain Quick System

BRAIN QUICK System is a technology reference point in the field of medical equipment for neurophysiology.

The multi-channel amplifier SD64 has very high input level (25.6 mV), the very high quantization resolution (22 bit) and the possibility to select the sampling rate that makes this amplifier absolutely ideal for scalp recording, intra-cranial depth electrode recording (Stereo EEG) and corticography.

 


 The fibre optic technology for data transmission to the PC guarantee the maximum patient safety and the total absence of artefact even when using very long cables to connect to the recording unit that can be thus positioned in the best possible position. The fibre optic can be connected directly to the headbox or via a thin cable 5 m. long to leave patient more mobility when the patient is "wearing" the headbox on himself.

Biodex Balance System

 The Biodex balance system is a tool to supplement balance evaluations and treatment for post-surgical patients or those with less than perfect balance. Athletes who are returning to highly coordinated & competive sports use the Biodex to best prepare for rapid reflex movements. It can also be used for anyone with high fall risk or balance dysfunction as a component of their treatment program.

 

 

 The computerized Biodex reports, direct the therapist to a clearer understanding of which quadrants of movement are the most challenging for each balance patient. Comparisons by age, gender & normative data are also made. Once this component

of balance is carefully evaluated a specific treatment program using the Biodex may significantly impact balance reactions. A variety of timed, weight shifting & balance recovery protocols may be used.

 

 

 The Biodex Balance System evaluates neuromuscular control by quantifying the ability to maintain dynamic postural stability on an unstable surface. 

 The degree of surface instability is controlled by a microprocessor-based actuator. The test consists of recording the patient's ability to control the platform variance from a perfectly balanced position. A large variance is indicative of poor control and balance. Further definition is realized with the quantification of Anterior/Posterior and Medial/Lateral response.

Tests are started with a completely firm surface. Progressing to a variety of stability levels, up to 20 degrees of support surface tilt, to ensure increased stimulation of joint mechanoreceptors.

 سیستم تعادل

به  جز تحقیقات مربوط به تعادل افراد در شرایط آزمایشگاهی ، سیستم تعادل در ارزیابی ، آموزش و درمان بیماران مبتلا به اختلالات نورولوژیک ، درمان های بعد از جراحی و به طور کلی در مواردی که تعادل افراد دچار اختلال شده باشد ، کاربرد پیدا می کند . مدل سیستم موجود در آزمایشگاه بیومکانیک ، Biodex  بوده و دارای یک صفحه متحرک با 8 درجه سفتی و به میزان 20 درجه تیلت در هر جهت می باشد.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Multi-Planar Ankle Perturbation System (APS)

  A laboratory setup for mechanical simulation of ankle sprain has been made. It consists of three sets (sagittal, frontal, and oblique) of dual-platforms. Each platform is like a trap-door to which the foot of subject is fixed. The perturbation angle is selectable at 10, 20, and 30 degrees. Each platform could be released independently from the pneumatic control panel by the examiner at a proper time.

 Onset of the platform tilt and EMG of muscles are directed to a real-time integrated hardware/software system for signal conditioning, data acquisition, and post processing.

 The preliminary results from the software are sequence of activation and of muscles and latencies of responses.

سیستم شبیه سازی مکانیکی پیچ خوردگی مچ پا (APS)

این سیستم از قابلیت رها شدن و ایجاد اغتشاش در سه صفحه ساجیتال ، فرونتال و عرضی ، در سه زاویه 10 ، 20و 30 درجه برخوردار است . امکان ثبت همزمان سیگنال EMG و بررسی زمان شروع فعالیت عضلانی نیز در این دستگاه فراهم شده است.
